Complaints, as well as all other papers submitted by pro se litigants must be submitted to the Pro Se Intake Unit. The complaint must include a caption, a statement of facts, the name and address of the litigant and the litigant’s signature. A complaint with an original signature, as well as two copies of the complaint and either the proper filling fee or an application to waive the filing fee must be submitted to the Pro Se Intake Unit.
